A Clinical Anlaysis for the Operative Indications of Peptic Ulcer Disease before and after Having Been Used Histamine Antagonist



Abstract
Administration of histamine antagonist was populized for peptic ulcer disease since 1980, and this study retrospectively reviewed and analysed with patients of peptic ulcer disease who were surgically treated at the Department of Surgery, College
of
Medicine, Kyung Hee University Hospital during the period of Jan. 1975 to Dec. 1979 which histamine antagonist was not used as Group A and of Jan. 1985 to Dec. 1989 which histamine antagonist was used as Group B. The analysis focused on the
defferences
in frequency of surgical indication and incidence of age.
@ES The results were as follows.
@EN 1) The ratio between male and female was 9.1% to 0.9% in male and female patients among 25 of a total 255 cases in Group! and 8.9% to 1.1% male and female patients among 32 of a total 260 cases in Group B. There was no differences between two
groups.
2) In the incidence of age was 3rd to 6th decade(77.2%), and 15.4% in 2nd decade, 5.8% in teen and 1.8% 7th decade in Group A. In the incidence of age, the 3rd decade to 6th decade(68.8%) and 19.5% in 2nd decade and 5.8% in 7th decade were
highest
age.
The difference of age incidence between two groups was increased in 2nd decade and 7th decade.
3) Among the cases of operation for peptic ulcer, the incidence of the perforation was 33.6% out of 280 cases in Group A and 45.2% out of 292 cases in Group B and the decreasing order of frequency was at the age of 3rd decade, 5th decade, and
2nd
decade in Group A and 2nd decade, 5th decade, and 3rd decade in Group B.
4) Since Histamine antagonist has been used, the numner of patients who were operated for peptic ulcer perforation was increased and the age incidence was decreased also which were statistically significant(p<0.05).
5) Among the cases of operation for peptic ulcer, bleeding was 28.9% out of 280 cases in Group A ans 21.2% out of 292 cases in Group B. The bleeding incidence was statistically decreased in Group B(p<0.05%).
6) Operative cases either intractable ulcer or obstruction of peptic ulcer were decreased however on statistical significance was observed.
